How do I pay for services?
We are currently considered in-network with Blue Cross Blue Shield PPO, United Health Care, Lyra EAP, and Blue Cross Blue Shield HMO (Little Company of Mary). We are considered an out-of-network provider for all other insurance plans, but we will gladly provide you with a receipt so that you may submit your sessions to your insurance for reimbursement. We currently accept cash or check, and payment is due at time of service.
If you require documentation such as FMLA paperwork, unemployment, report writing etc. you will be charged my prorated rate of $130 hour for the amount of time it takes to complete the services.
How frequently should I attend therapy sessions?
The frequency of sessions will be determined based on clinical need and presenting symptoms. In most cases, I see patients weekly for the first 4 – 6 weeks. After that time we reevaluate your progress and determine the ongoing frequency of treatment. We will work together to create a treatment plan that fits your needs and addresses your individual goals
Hours & Scheduling
I see patients by appointment only. My hours are as follows:
Cancellation Policy/Missed Appointments
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $75 missed session fee. If this happens on more than one occasion, you will be charged the full fee of $130.
