We are currently considered in-network with Blue Cross Blue Shield PPO and Lyra EAP. We are considered an out-of-network provider for all other insurance plans, but we will gladly provide you with a receipt so that you may submit your sessions to your insurance for reimbursement. We currently accept cash or check, and payment is due at time of service.
If you do not have insurance or choose not to use your insurance we also accept private pay.
Ashley Hartman LCSW PMHC Prices:
Initial Psychiatric Evaluation - $225/ hour
Individual/Follow up Session (60 minutes) - $200/ hour
Ketamine Assisted EMDR (KAP) Session - $250 (90 minutes)
*Insurance does not cover Ketamine Assisted Psychotherapy*
Dana Jay LCPC & Sarah Forde LPC
Initial Psychiatric Evaluation - $200/ hour
Individual/Follow up Session (60 minutes) - $150/ hour
If you require documentation such as FMLA paperwork, unemployment, report writing etc. you will be charged my prorated rate of $200/ hour for Ashley Hartman or $150/ hr for other clinicians for the amount of time it takes to complete the services.
The frequency of sessions will be determined based on clinical need and presenting symptoms. In most cases, We see patients weekly for the first 4 – 6 weeks. After that time we re-evaluate your progress and determine the ongoing frequency of treatment. We will work together to create a treatment plan that fits your needs and addresses your individual goals.
Each therapist sets their own schedule for virtual and in person days. Please contact the clinician individually to determine which days they are in person and virtual.
